There will be no charge for corrective repairs if we determine the problem is covered under the Meis Warranty.

After review of this form or upon site investigation, if we determine the problem is not covered by our warranty, charges may be assessed for any service work, leak detection, temporary or permanent repairs that are completed.

You will be billed for all non-warranty services completed. If said service invoices are not paid within 30 days, Meis reserves the right to void our warranty on your project.

To avoid multiple mobilization charges in the event my problem is determined not to be covered under warranty, I pre-authorize Meis to make corrective repairs to address the problem described above up to a dollar amount not to exceed:
